Good Faith Estimate Notice

You have the right to receive a Good Faith Estimate detailing the cost of your medical care if you are uninsured or not using insurance. This estimate includes all expected charges for non-emergency services, such as medical tests, prescriptions, equipment, and hospital fees. Your healthcare provider must provide a written Good Faith Estimate at least 1 business day before your service. You can also request an estimate before scheduling any service or item.
